Dr. Fawcett is out-of-network for insurance companies and does not participate in medicare. Payment is due at the time of the appointment. Patients can request a superbill (including diagnosis codes and procedure codes) to submit to their insurance to be reimbursed for out-of-network costs, according to their insurance plan.

If you're seeking reimbursement for out-of-network care we will provide you with a detailed superbill that includes all the necessary information, such as the services provided, diagnosis codes, and the amount paid. You can submit this superbill to your insurance company for reimbursement according to your out-of-network benefits. Depending on your insurance plan, you may be reimbursed for 0-100% of the cost depending on your out-of-network coverage plan.
